Evaluation
The dentist will summarize your medical information and dental concerns to verify the accuracy of the discussed issue. If further information is needed, it will be addressed in this section.
Findings
Dental issues such as decay, periodontal findings, failing crowns, tooth fractures, or other pertinent findings will be documented and explained so you fully understand the problems. The dentist may use dental terminology in this section, so refer to the common dental terms glossary below.
Assessment
